The CCFT Case Manager will provide primary case-management for an assigned group of clients, including on-going assessment, rehabilitation and support services. Completes intake, assessment, job placements and coaching. Works closely with IOP treatment teams to place consumers in competitive, diverse jobs that are tailored to meet the consumer’s individualized needs and preferencesCase Manager (Health Link 910) Hamblen
The Health Link Care Coordinator uses an integrative approach to interface with agencies increasing cooperation and consistency between home, school, work, community, and behavioral and physical health providers as defined by the individuals Care Plan. Provides crisis intervention and emergency services, as well as utilizes Center-wide and community resources as needed for client benefit. Provides transportation to clients as needed to meet the goals of their Care Plan.Case Manager (Recovery Home) Hamblen
The Recovery Home Case Manager will serve as primary case manager that provides case coordination using a social model approach to a predetermined number of adult women in recovery.Peer Support Specialist (CSU) Hamblen
The Peer Support Specialist position is intended to provide hope for clients by having a support person who is or has been a primary consumer of mental health or addiction services and is willing to share their experiences and story with mental illness, treatment, and recovery; to be a role model for clients who are currently in crisis and at risk for incarceration.
