In preparation for the SIM Mapping Workshop Policy Research Associates, Inc.
requests you participate in a Community Self-Assessment survey. The purpose of the
Community Self-Assessment is to ascertain a community’s level of
collaboration and activities relating to justice-involved adults with mental disorders
and/or substance use disorders. This Community Self-Assessment is
intended to guide identification of opportunities for improving
responses to justice-involved adults along the intercept points of the
Sequential Intercept Model. The survey is estimated to take
less than 30 minutes to complete.
